Directions

From the Bourne Bridge:

• Once over the bridge, take the MA-28 S/General MacArthur Blvd exit
• At the Otis Rotary, take the MA-28 South exit
• Take the MA-151 exit toward Mashpee/Falmouth and continue onto MA-151 E
• Take a right onto Jobs Fishing Rd, then a second right onto MA-28 N
• At the traffic lights, take a left onto Charles St and a right onto Commercial St Community Health Center will be on the right

From Hyannis

• Take MA-28 N/Falmouth Rd toward Falmouth/Mashpee
• At the traffic circle, take the Great Neck Rd S exit
• Turn right onto Mercantile Way, then right onto Commercial St
• Community Health Center will be on the left

By bus:

The Breeze/Sealine bus, which runs between Woods Hole and Hyannis on a schedule, will stop at Community Health Center, if you ask the driver. If you’re at Community Health Center, call 800-352-7155 for pick-up. Be sure to check the schedule before making a late afternoon or evening appointment at Community Health Center.  For more information or schedules, visit www.capecodtransit.org.
