From Sagamore Bridge

• Take US 6 E to exit 55 toward MA-6A/Sagamore
• Continue onto Mid-Cape Connector
• Turn right onto Sandwich Rd
• Continue onto MA-6A East
• Community Health Center is on the right within Sextant Hill complex. You can use Webster Avenue to access Sextant Hill – the complex is located next to Village Florist and diagonally across from Cape Cod 5

From Mashpee

• Take 130 N toward Sandwich
• Turn right onto Tupper Rd
• Take a left at the intersection with MA-6A
• Community Health Center is on the left within the Sextant Hill complex.  You can use Webster Avenue to access Sextant Hill – the complex is located next to Village Florist and diagonally across from Cape Cod 5
