Primary Care

Taking Care of Your Whole Family

There’s nothing more important than having the peace of mind to know that you and your family are healthy.

That starts with having access to the very best Primary Care services — for adults, children and entire families — delivered by an experienced and compassionate care team that truly gets to know you and provides outstanding care and an exceptional patient experience.

CHC is a Patient Centered Medical Home where your primary care provider can manage as much of your health care under one roof as possible. We encourage patients to be true participants in their medical care and believe this leads to the best health outcomes for those we serve.

Primary Care services at Community Health Center include:

  • Medical care
  • Walk-in care
  • Regular check-ups
  • Physicals
  • Health screenings
  • Immunizations
  • School physicals and screenings
  • Diagnostic testing
  • Patient education

Community Health Center offers Walk-in care in Mashpee, Bourne and Centerville. Walk-in services are available for anyone in the community who needs them, and are for urgent needs such as flu, cough, cold, sore throats and other minor illness or injury.

If you need help with insurance, our specially trained staff will assist you in applying for MassHealth, new federal health plans and other public benefit programs.

Our team helps take care of all your healthcare needs — whether it is for routine care, dealing with a chronic illness or something more complex. We provide case management and patient navigation services to coordinate any of your health needs. We also assist with family planning, including annual gynecological and breast exams, screening and treatment for sexually transmitted diseases and HIV, pregnancy testing and information. Outpatient addiction medicine services are available for both opiate and alcohol dependence. Nutrition counseling with a registered dietician is also available to help in managing issues related to diabetes, congestive heart failure and other chronic illnesses.

And, if you should need prescription medications, our onsite pharmacies are right here to provide low- or no-cost medications.

We even have foreign language and sign language interpreters available, should you need them.

We know your health is so important — and we believe that preventing illness is a priority. We emphasize good nutrition, the importance of physical activity and regular check-ups to promote health and well-being.
