NEW- Patient Access Representatives II (Family Medicine/General Medicine)

  • Completes checkout including review of after visit summary; labs, referrals and other orders; schedules primary care follow up visit and visits with CHC specialty services
  • Completes and documents New Patient orientation in chart

NEW- Community Health Worker/Complex Care

The CHW will work closely with the Complex Care Manager, patients, and the primary care teams to effectively manage the CHC of Cape Cod’s highest- risk patients.

  • Work proactively to remove barriers that impede access to healthcare and prevent successful implementation of the patient care plan
  • Provide culturally appropriate individual and group health education; teach self-management and lifestyle skills.  Educate clients on available community resources
  • Responsible for documentation of appropriate patient interactions. Responsible for helping to update high risk patient lists, and participating in the creation of patient care plans with the CCM.

EXPERIENCE/EDUCATION: High school diploma with advanced coursework or related training a plus. Prior experience in health, social or community services required. Experience working with low-income, ethnically diverse individuals and communities is considered a plus. Comfort working with patients with chronic disease as well as their caregivers. Strong organizational skills required; demonstrated ability to set goals and prioritize workflow as well as proficient computer skills.

Registered Nurse (Walk-In Services)

Pharmacist In-Charge / Bourne Offices

CHC of Cape Cod is seeking a Pharmacist In-Charge for the Bourne office location who is responsible for overall daily operations and supervision of the pharmacy department,  including, but not limited to, operational and clinical services. Such services include supervising staff, purchasing, preparation and distribution of drugs; clinical oversight and interaction with medical staff; and oversight of appropriate/timely submission/reconciliation of drug claims.

EXPERIENCE/EDUCATION: A Bachelors, Masters or Doctoral degree in Pharmacy from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).  A current, valid, unrestricted license to practice Pharmacy in the Commonwealth of Massachusetts. Must have 1-3 years pharmacy or related experience, preferably supervisory in outpatient clinic pharmacy services or retail pharmacy. Must be detailed oriented and possess good people skills. Must possess both written and verbal communication skills in order to interact with patients and clinic staff.

Experienced dental hygienist needed to assist dentists in providing direct oral health care to patients in a dental clinic setting:

CHC of Cape Cod Development Office Internship/Volunteer Opportunity

CHC of Cape Cod is seeking a Development Office Intern to help us achieve our mission by supporting communications, event planning, and marketing for our upcoming events and general CHC of Cape Cod fundraising.

Job responsibilities will include, but may not be limited to:

  • Help Development team with marketing for fundraising events
  • Support Foundation research efforts
  • Explore third party fundraising ideas
  • Research possible education and outreach events and contacts
  • Solicit auction item/raffle donations
  • Assist in database entry
  • Online research and Social Media
  • Support additional general Development activities
  • Support other Health Center initiatives and offices, as needed

EXPERIENCE/EDUCATION: High School degree preferred. Some completed College coursework preferred. Interest and familiarity with Social Media Applications/ Marketing/ Data Entry and Nonprofit Fundraising a plus. Must be motivated, reliable, and independent with good time-management and communications skills.

8-10 hours per week based on availability. Hours can be flexible based on interest and availability. Desired timeframe is June – August, but can be adjusted as needed.
