Care is provided by a team of healthcare professionals working together to provide coordinated care for all your health needs. Our providers have varying expertise and training to take care of a wide range of medical, mental health and dental needs.
Medical care is provided by physicians, nurse practitioners or physician assistants. Nurse practitioners and physician assistants are advanced healthcare providers who work under the direction of a physician. They perform physical exams, diagnose and treat acute problems, order diagnostic tests, and prescribe medications.
Mental health services are provided by psychiatrists, counselors and social workers. Dentists complete the clinical team.
These providers are supported by nurses, medical assistants, hygienists, enrollment, registration and clerical staff.
DAVID S. TAGER, MD, CHIEF MEDICAL OFFICER
As Chief Medical Officer at the Health Center, Dr. Tager plans and directs all aspects of the Health Center’s medical policies and programs and oversees the quality improvement process. He also sees patients. Dr. Tager is a Phi Beta Kappa graduate of Cornell University and received his medical degree from New York University School of Medicine. He completed a residency in internal medicine at The Miriam Hospital – Brown University, in Providence, RI and a fellowship in gastroenterology at Lahey Clinic Medical Center in Burlington, MA. He is board certified in internal medicine and gastroenterology. Prior to the Health Center, Dr. Tager was in private practice in Rutland, VT, where he also served as Medical Director of Eden Park Nursing Home.
Medical Providers:
Yves-Line Auguste-Swann, MD
Dr. Swann received a medical degree from Albert Einstein College of Medicine. She completed an internship in internal medicine at Yale University School of Medicine, where she also served as chief resident and clinical instructor. She received a Master’s in Public Health degree in family and community health from the Harvard School of Public Health. Before joining the Health Center, Dr. Swann worked at Cambridge Health Alliance. She is board-eligible in internal medicine.

Ronald Backer, MD
Dr. Backer is an internist/family physician with 33 years of experience in private practice and health administration. He received his medical degree from Boston University School of Medicine and completed his residency training at Mt. Auburn Hospital in Cambridge, MA.

Kristy Brown, DO
Dr. Brown received a bachelor’s of science degree in psychology from the University of Massachusetts, Amherst and a medical degree from the University of New England College of Osteopathic Medicine in Biddeford, Maine. She completed her residency in family medicine and served as chief resident at the University of Massachusetts Medical Center in Worcester. She was assistant professor, family medicine at the University of Massachusetts Medical Center in Worcester; clinical lecturer, family medicine at the University of Michigan Medical School in Ann Arbor, MI; and assistant residency director, family medicine at the University of Michigan Medical School, Ypsilanti Family Practice Clinic in Ypsilanti, MI. Dr. Brown is board certified in family medicine.
Jean Canty-Schwartz, RN, MS, CPNP
Ms. Schwartz is a certified pediatric nurse practitioner. She holds a Master’s of Science degree in pediatric ambulatory medicine from Boston College and a Bachelor’s of Science degree in nursing from Boston College. She received her nursing degree from Children’s Hospital School of Nursing. Before coming to the Health Center, Ms. Schwartz was in private pediatric practice on the Cape. Her additional experience includes working as a staff nurse in hospital, home care and other community settings. She also is a nursing professor at Cape Cod Community College.
Megan Cavanaugh, ANP
Ms. Cavanaugh is an adult nurse practitioner. She received a Bachelor of Science degree in biology from Pennsylvania State University and Master of Science degree in nursing from Simmons College. She has experience in a variety of clinical settings including community health.

Kyle Chaplic, FNP
Mr. Chaplic is a family nurse practitioner. He received a Bachelor of Arts degree in music from Westfield State University and Master of Science degree in nursing from Yale School of Nursing. During his training, he completed clinical rotations in family and community health, primary care pediatrics, emergency medicine, family practice, and pediatric urgent care. Before joining the Health Center, he worked as a nurse in various clinical settings.
Beth Homand, NP
Ms. Homand, who works with the Falmouth Family Planning program, is certified as an OB/GYN nurse practitioner. She received her bachelor of science in nursing and her nurse practitioner training from Boston College. Prior to the joining the Health Center team, she worked at Harvard Vanguard Medical Associates and other Boston–area hospitals.
Karen Hussion, ANP-BC
Ms. Hussion is a board certified adult nurse practitioner, with advanced geriatric education. She received her registered nursing degree from Riverside Hospital School of Professional Nursing in Newport News, VA; her Bachelors of Science in Nursing from Framingham State College in Framingham, MA; her Masters of Science in Nursing and adult nurse practitioner training at University of Massachusetts at Dartmouth; and certificate of advanced graduate study in geriatrics from University of Massachusetts at Boston. Ms. Hussion most recently worked in private medical case management and at a private medical group on Cape Cod.
Matthew Masiello, MD, MPH
Dr. Masiello has more than 25 years experience as a pediatrician, wellness and public health professional. During this time, he has organized extensive community-based efforts aimed at bullying, childhood obesity, and the impact of gun violence on children. He previously served as Chairman of the Department of Pediatrics, Medical Director of Woman/Child Services, and Director of the Office of Community Health at Memorial Medical Center in Johnstown, PA.
Dr. Masiello received his medical degree from Autonomous University of Guadalajara Medical School. He completed his residency in pediatrics at Bridgeport Hospital in Bridgeport, CT; and a fellowship in pediatric intensive care at the Children’s Hospital, Harvard Medical School. He also holds a master’s degree in public health from the George Washington University School of Public Health and Health Services.
Dr. Masiello also is Chief Medical Officer/Chief Wellness Officer and Director of the Center for Health Promotion and Disease Prevention at the Windber Research Institute in Windber, PA. He also serves on the governing board of the World Health Organization and as project coordinator for its international Health Promoting Hospital Network.
Allegra McElroy, FNP
Mrs. McElroy is a family nurse practitioner. She is a registered nurse with advanced education and clinical training. She received her nurse practitioner training at Massachusetts General Hospital, Institute of Health Professions, in Boston. Mrs. McElroy’s specialty is family medicine and she sees pediatric and adult patients as part of the Health Center’s Family Care program.
Anne Pelletier, NP
Ms. Pelletier is a nurse practitioner. She received a Bachelor’s of Science degree in nursing from Boston University and a Master’s of Science degree in nursing from Boston College. She has worked in several clinical settings, including private practice, long-term care, rural acute hospitals and Massachusetts General Hospital as well as in various healthcare management positions.
Stephanie Prior, MD
Dr. Prior, a board certified family practice physician, has many years experience in the community health center setting. She obtained her medical degree from the University of Massachusetts Medical School and received her family practice training at Pawtucket Memorial Hospital in Rhode Island, part of the Brown University Family Medicine Program. Before joining the Health Center staff in April, 2006, Dr. Prior was in private practice in Bourne. She was also a physician volunteer at the Health Center.
Jennifer Ring, PA-C, MS
Ms. Ring received a Bachelor of Science degree in biology from Seton Hall University. She obtained a Master of Science in physician assistant studies from Seton Hall University, where she was a member of Pi Alpha Honor Society for physician assistants. Prior to joining g the Health Center, she was a physician assistant in the emergency room at St. Luke’s Hospital in New Bedford.
Lisa M. Taylor, MD
Dr. Taylor is a board certified internal medicine specialist. She is a graduate of the Robert Wood Johnson Medical School in Piscataway, NJ. She completed her residency at Christiana Hospital in Wilmington, DE, where she was chief resident. Dr. Taylor was recently named 2006 Community Clinician of the Year by the Barnstable District Medical Society of the Massachusetts Medical Society.
Sophie Thibodeau, WHNP-BC
Ms. Thibodeau is a women’s health nurse practitioner. She received a Bachelor of Arts degree in political science from Boston College, a Master of Arts degree in anthropology, and a Master of Science degree in Nursing from Boston College School of Nursing. During her training, she worked in an OB-GYN office, a college health center, a community health center, and in a mobile clinic in Haiti. She has completed research in Type II diabetes and in health disparities in immigrant and non-immigrant women.
Phyllis Troia, MD
Dr. Troia is board certified in internal medicine and emergency medicine. She received a Doctor’s of Medicine degree from the University of Massachusetts Medical School and a Bachelor’s of Science degree in biology from Providence College. She also completed post-graduate work in biochemistry at Boston University School of Medicine. Dr. Troia completed residencies at Albany Medical Center and Mount Sinai Hospital. She has worked as an emergency medicine physician and in various internal medicine settings, and as a physician consultant.
John Yeh, MD
Dr. John Yeh is board certified in obstetrics and gynecology and in reproductive endocrinology. He currently serves as a Professor of Obstetrics and Gynecology at Harvard Medical School and is based at Massachusetts General Hospital. He has been honored as one of America’s Top Obstetricians and Gynecologists and as one of America’s Top Doctors. Dr Yeh received his undergraduate degree in biochemistry from Harvard University and his medical degree from the University of California, San Diego School of Medicine. He completed his residency in obstetrics and gynecology at Beth Israel Deaconess Hospital, Harvard Medical School and was a clinical fellow at Brigham and Women’s Hospital, Harvard Medical School.
Behavioral Health Providers:
Ralph Cahoon, III, MSW
Mr. Cahoon is a counselor. He obtained an Associate’s in Arts degree from Cape Cod Community College, his Bachelor’s of Science degree in law enforcement from Western New England College, and a Master’s of Social Work degree from Boston University. Mr. Cahoon worked for many years as a patrol officer and accreditation manager at Barnstable Police Department. He completed social work internships at CHC and at Gosnold on Cape Cod.

Maya Carpenter, LMHC
Ms. Carpenter is a counselor at the School Based Health Center. She is certified in school guidance and school adjustment/social work. Ms. Carpenter has broad experience working with children, adolescents and young adults with mental health and substance abuse diagnoses and providing individual, family and group therapy in conjunction with assessment, clinical diagnosing and treatment planning. She has varied experience in the school setting, as a guidance counselor and working with at-risk students in alternative learning settings. Ms. Carpenter was awarded a masters degree in counseling psychology from Boston College School of Education and a bachelors degree in psychology from Wheaton College.

Alice Cook, LICSW, MBA, LADC I – Director of Behavioral Health
Ms. Cook is a clinical social worker and alcohol and drug counselor. She has a variety of experience in community-based behavioral health and substance abuse settings and has held positions with several human service organizations on Cape Cod and the Islands. Throughout her career she has blended both direct practice and management responsibilities. She began her career at a settlement house in the inner city of Chicago where she provided diverse services to an immigrant community. She has worked in day care, with child maltreatment issues and the Juvenile Court, homeless families, and Alzheimer’s patients. Her behavioral health experience includes treatment with victims of trauma and sexual abuse, issues with domestic violence, working with a culturally diverse population as well as managing a behavioral health clinic. Fluent in Spanish, she is a certified bi-lingual medical interpreter. Ms Cook obtained a Bachelor of Arts degree in sociology from the University of Illinois and a Master of Social Work degree from Loyola University. She also holds a Master of Business Administration degree from Babson College.
Jayne Farley, MD
Dr. Farley is a board-certified psychiatrist. She received her bachelors degree in psychology from Bucknell University in Lewisburg, PA and her medical degree from Medical College of Georgia in Augusta, GA. She completed a residency in psychiatry at University of Connecticut School of Medicine in Farmington, CT and a fellowship in psychotherapy at Harvard University Health Services in Cambridge, MA. Dr. Farley worked as an attending psychiatrist at Norwood Hospital and Mt. Auburn Hospital. In addition to her position at the Health Center, she has a private practice in Falmouth.
LaRue Holteen, RNCS
Ms. Holteen is a registered nurse and holds a Masters of Education in counseling psychology. She is certified by the American Nurses Association as a psychiatric clinical nurse specialist. She began as a volunteer and joined the part-time staff in 2005. Prior to the Health Center she worked at the Veterans Hospital in Bedford, MA.
James P. McGuire, MD
Dr. McGuire is a child psychiatrist with the Family Resource Center. He received his medical degree from Boston University School of Medicine and completed residencies in psychiatry at University of Cincinnati, University of Cincinnati Medical Center; University of Pennsylvania, Philadelphia Child Guidance Clinic at Philadelphia General Hospital; and Brown University, Butler Hospital. Dr. McGuire, who is board certified in psychiatry and child and adolescent psychiatry, is also on the staff at Gosnold/Thorne Counseling Center.
Linda Porto, MSN, PMHCNS-BC
Ms. Porto is a nurse practitioner in psychiatry and certified by the American Nurses Association as a clinical nurse specialist. She received her nursing degree from Misericordia Hospital School of Nursing, Bronx, NY; a bachelor of science in community mental health from State University of New York, Empire State College; masters of science in nursing, psychiatric mental health from Adelphi University, Garden City, NY; and wholistic nursing certification from New York College for Health, Education and Research, Syosset, NY. She has extensive experience in mental health nursing and working with children and adolescents. She has also served on the faculty of schools of nursing at University of Southern Alabama, Boston College and Adelphi University.
Kenneth Terkelsen, MD
Dr. Terkelsen is a psychiatrist. He is a graduate of Georgetown University and received his medical degree from Jefferson Medical College of Thomas Jefferson University in Philadelphia. He completed a residency in psychiatry at Albert Einstein College of Medicine at Lincoln Hospital and a fellowship in social and community psychology at Albert Einstein College of Medicine at Bronx Psychiatric Center. Board certified in psychiatry, Dr. Terkelsen has many years of experience in private practice and in community settings on Cape Cod including Gosnold-Thorne Clinics, Cape Cod Human Services/Cape Cod Behavioral Healthcare and the Cape Cod and Islands Community Mental Health Center.
Kaye Wagner, MSW
Ms. Wagner is a counselor. She received a Bachelor’s of Science degree in sociology/criminal justice from the University of Massachusetts and a master’s degree in clinical social work from Boston University. She completed social work internships at CHC and Gosnold on Cape Cod. Before joining the Health Center, she worked at the Commission on Family – Transitional Housing program for the City of Quincy.
Katherine Walsh, LMHC
Ms. Walsh is a counselor. She has a Masters degree in Expressive Therapies from Lesley University. She previously worked as a clinician at Gosnold Treatment Center and as a school based counselor. Ms. Walsh has also worked in day treatment and partial hospitalization settings facilitating various therapy groups.
Glenn Williams, RN
Mr. Williams has an Associate of Science in Nursing degree from Mt Wachusett Comm College in Gardner, MA. He has 33 years experience in various psychiatric nursing settings including the Visiting Nurse Association, Massachusetts Department of Mental Health acute and chronic care settings, private psychiatric hospital inpatient units, hospital emergency rooms, and crisis intervention sites. He has expertise with dual diagnosis and detox inpatient care.
Heidi Wright, LMHC
Ms. Wright obtained her Masters degree in counseling psychology from Lesley University. She has worked for Massachusetts Society for the Prevention of Cruelty to Children doing outreach with clients ages 4 thru adult and has extensive experience working with those affected by trauma. She has also worked with inmates at the Barnstable County House of Correction.

Joseph Flaherty, III, DMD
Dr. Flaherty obtained a Doctor’s of Dental Medicine degree from Boston University School of Dental Medicine and a Bachelor’s of Science degree in biochemistry from Boston College. He completed a general practice residency at Penobscot Community Dental Clinic.
Harold Goldstein, DDS
Dr. Goldstein received a dental degree from Temple University School of Dentistry and completed his undergraduate degree at Brown University. He was in private practice before joining the Health Center.
Amelia Grabe Lane, DMD
Dr. Lane received a Doctor’s of Dental Medicine degree from Boston University School of Dental Medicine a Bachelor’s of Arts degree from Hamilton College. She has also completed post-graduate coursework at Harvard University and the University of New Hampshire. Before joining the health Center she was in private practice. Dr. Lane is active in Massachusetts Dental Society and Cape Cod District Dental Society.
Narcis Popita, DMD
Dr. Popita received a dental degree from the University of Oradea School of Medicine and Pharmacy in Romania and a degree in biology and chemistry from College H.C.C. in Romania. His experience includes working in an emergency room, private practice and in the community health center setting.
Jules Rosenberg, DDS
Dr. Rosenberg received a dental degree from New York University College of Dentistry. Prior to joining the Health Center, Dr. Rosenberg was in private practice and was also a clinical instructor at the Dental Hygiene Program at Cape Cod Community College.
Daniel Schafft, DMD
Dr. Schafft obtained a Bachelor of Science degree in Chemistry from the University of Central Florida. He received a degree in dental medicine from the Nova Southeastern University College of Dental Medicine and completed a residency at Harvard School of Dental Medicine, Cambridge Hospital. He has worked in various settings providing comprehensive dental treatment for patients of all ages.
Paul Spinale, DDS
Dr. Spinale received a dental degree from the University of Medicine and Dentistry of New Jersey. He was in private practice before coming to the Health Center.

Dawn West, DMD, Director of Dental Services
Dr. West earned a bachelor of science degree in nursing at Skidmore College in Saratoga Springs, NY. She received a doctor of dental medicine degree from Boston University Goldman School of Dental Medicine. Dr. West has extensive experience in dental public health and community services with a concentration in medicine. Prior to joining the Health Center, she was a dentist at community health centers and hospitals in the region and also directed the dental residency program at Boston Medical Center. She is on the faculty of Tufts University School of Dental Medicine. |
