MEET OUR HEALTHCARE PROVIDERS

Care is provided by a team of healthcare professionals working together to provide coordinated care for all your health needs. Our providers have varying expertise and training to take care of a wide range of medical, mental health and dental needs.

Medical care is provided by physicians, nurse practitioners or physician assistants. Nurse practitioners and physician assistants are advanced healthcare providers who work under the direction of a physician. They perform physical exams, diagnose and treat acute problems, order diagnostic tests, and prescribe medications.

Mental health services are provided by psychiatrists, counselors and social workers. Dentists complete the clinical team.

These providers are supported by nurses, medical assistants, hygienists, enrollment, registration and clerical staff.

DAVID S. TAGER, MD, CHIEF MEDICAL OFFICER

Dr. TagerAs Chief Medical Officer at the Health Center, Dr. Tager plans and directs all aspects of the Health Center’s medical policies and programs and oversees the quality improvement process. He also sees patients. Dr. Tager is a Phi Beta Kappa graduate of Cornell University and received his medical degree from New York University School of Medicine. He completed a residency in internal medicine at The Miriam Hospital – Brown University, in Providence, RI and a fellowship in gastroenterology at Lahey Clinic Medical Center in Burlington, MA. He is board certified in internal medicine and gastroenterology. Prior to the Health Center, Dr. Tager was in private practice in Rutland, VT, where he also served as Medical Director of Eden Park Nursing Home.

Medical Providers:

Mary AgnewMary Agnew, FNP
Ms. Agnew is a family nurse practitioner. She received her associates degree in nursing from Cape Cod Community College and a masters in science, nursing from Massachusetts College of Pharmacy and Health Sciences. Prior to joining the Health Center, she worked as a primary care provider at a practice in Harwich. Ms. Agnew also serves as a clinical advisor/preceptor for nurse practitioner students at Massachusetts General Hopsital School of Nursing.

 

Donald AngelettiDonald Angeletti, MD
Dr. Angeletti is a board-eligible endocrinologist. He received his undergraduate degree from Dickinson College in Carlisle, PA and completed postgraduate studies at Long Island University and Seton Hall University. He received his medical degree from the University of Bologna School of Medicine and Surgery in Bologna, Italy. Dr. Angeletti completed an internal medicine residency at Mountainside Hospital in Montclair, NJ and an endocrinology and metabolism residency at East Orange Veterans Hospital in East Orange, NJ. Prior to joining the Health Center, he was in private practice in New Jersey.

Tara ArmstrongTara Armstrong, PA-C, MMS
Ms. Armstrong is a physician assistant. She most recently worked as an internal medicine provider at a practice in Sandwich and prior to that at a community health center in Manchester, NH. Ms. Armstrong received her bachelor of science, biology from Providence College and her master of medical science and physician assistant certification from Midwestern University in Downer’s Grove, IL.

 

Dr. Backer

Ronald Backer, MD
Dr. Backer is an internist/family physician with 33 years of experience in private practice and health administration. He received his medical degree from Boston University School of Medicine and completed his residency training at Mt. Auburn Hospital in Cambridge, MA.

 

 

Dr. CohenRobert Cohen, MD
Dr. Cohen, who moved to the Cape from Middlebury, Connecticut, is a board certified internist with many years of experience in private practice and in the US Army Medical Corp. He received his undergraduate degree from Union College in Schenectady, NY and his medical degree from the University of Pennsylvania School of Medicine. He completed his residency in internal medicine at Bronx Municipal Hospital Center, affiliated with Albert Einstein College of Medicine, and also completed a fellowship in pulmonary medicine.

Dr. HallemeierAnna Hallemeier, MD
Board certified in both internal medicine and pediatrics, Dr. Hallemeier sees adults and pediatric patients. She is particularly interested in working with teens and “helping them make the right behavioral choices that will affect the rest of their lives”. She received her medical degree from Yale University School of Medicine and completed her residency in medicine and pediatrics at Yale-New Haven Hospital in New Haven, CT. Prior to joining the Health Center, Dr. Hallemeier worked as a pharmaceutical consultant and on the internal medicine faculty at Yale.

Beth HomandBeth Homand, NP
Ms. Homand, who works with the Falmouth Family Planning program, is certified as an OB/GYN nurse practitioner. She received her bachelor of science in nursing and her nurse practitioner training from Boston College. Prior to the joining the Health Center team, she worked at Harvard Vanguard Medical Associates and other Boston–area hospitals.

Karen HussionKaren Hussion, ANP-BC
Ms. Hussion is a board certified adult nurse practitioner, with advanced geriatric education. She received her registered nursing degree from Riverside Hospital School of Professional Nursing in Newport News, VA; her Bachelors of Science in Nursing from Framingham State College in Framingham, MA; her Masters of Science in Nursing and adult nurse practitioner training at University of Massachusetts at Dartmouth; and certificate of advanced graduate study in geriatrics from University of Massachusetts at Boston. Ms. Hussion most recently worked in private medical case management and at a private medical group on Cape Cod.

Allegra McElroyAllegra McElroy, FNP
Mrs. McElroy is a family nurse practitioner. She is a registered nurse with advanced education and clinical training. She received her nurse practitioner training at Massachusetts General Hospital, Institute of Health Professions, in Boston. Mrs. McElroy’s specialty is family medicine and she sees pediatric and adult patients as part of the Health Center’s Family Care program.

 

Shane PetersShane Peters, APRN-BC
Ms. Peters is a family nurse practitioner. She received her undergraduate degree from Providence College, in Rhode Island and obtained a masters degree in nursing and nurse practitioner training at Regis College in Weston, Massachusetts. Prior to joining the Health Center, she was in private practice on Nantucket and also worked with the Nantucket AIDS Network.

 

Dr. PriorStephanie Prior, MD
Dr. Prior, a board certified family practice physician, has many years experience in the community health center setting. She obtained her medical degree from the University of Massachusetts Medical School and received her family practice training at Pawtucket Memorial Hospital in Rhode Island, part of the Brown University Family Medicine Program. Before joining the Health Center staff in April, 2006, Dr. Prior was in private practice in Bourne. She was also a physician volunteer at the Health Center.

Dr. TaylorLisa M. Taylor, MD
Dr. Taylor is a board certified internal medicine specialist. She is a graduate of the Robert Wood Johnson Medical School in Piscataway, NJ. She completed her residency at Christiana Hospital in Wilmington, DE, where she was chief resident. Dr. Taylor was recently named 2006 Community Clinician of the Year by the Barnstable District Medical Society of the Massachusetts Medical Society.

 

 

Dale Weldon MDDale Weldon, MD
Dr. Weldon is board certified in obstetrics and gynecology. She has been in private practice on the Upper Cape for several years and joined the Health Center to establish the Women’s Health service. She received her medical degree from Emory University School of Medicine in Atlanta and completed her residency in OB/GYN at Emory University Affiliated Hospitals.

Behavioral Health Providers:

Lee BurwellLee Burwell, LMHC
Ms. Burwell is a counselor with the Family Resource Center. She earned a degree in Family Therapy from Antioch Graduate School in Keene, NH and has extensive experience working with teen mothers and families. Prior to joining the Health Center, she worked at Falmouth Human Services and in private practice. She also teaches parenting classes for the Coalition for Children and Bringing Baby Home classes at Falmouth Hospital.

Emily Davern

Emily Davern, LICSW, Asst. Director of Behavioral Health
Ms. Davern is a is a clinical social worker with our Family Care program and Family Planning. She has extensive experience working with disenfranchised families impacted by domestic violence and has worked as a program developer of statewide programs, including fiscal oversight and evaluation planning. She is a graduate of Boston College School of Social Work. She is also a member of the Falmouth School Committee.

Jayne FarleyJayne Farley, MD
Dr. Farley is a board-certified psychiatrist. She received her bachelors degree in psychology from Bucknell University in Lewisburg, PA and her medical degree from Medical College of Georgia in Augusta, GA. She completed a residency in psychiatry at University of Connecticut School of Medicine in Farmington, CT and a fellowship in psychotherapy at Harvard University Health Services in Cambridge, MA. Dr. Farley worked as an attending psychiatrist at Norwood Hospital and Mt. Auburn Hospital. In addition to her position at the Health Center, she has a private practice in Falmouth.

LaRue HolteenLaRue Holteen, RNCS
Ms. Holteen is a registered nurse and holds a Masters of Education in counseling psychology. She is certified by the American Nurses Association as a psychiatric clinical nurse specialist. She began as a volunteer and joined the part-time staff in 2005. Prior to the Health Center she worked at the Veterans Hospital in Bedford, MA.

James P. McGuire, MD James P. McGuire, MD
Dr. McGuire is a child psychiatrist with the Family Resource Center. He received his medical degree from Boston University School of Medicine and completed residencies in psychiatry at University of Cincinnati, University of Cincinnati Medical Center; University of Pennsylvania, Philadelphia Child Guidance Clinic at Philadelphia General Hospital; and Brown University, Butler Hospital. Dr. McGuire, who is board certified in psychiatry and child and adolescent psychiatry, is also on the staff at Gosnold/Thorne Counseling Center.

Linda PortoLinda Porto, MSN, PMHCNS-BC
Ms. Porto is a nurse practitioner in psychiatry and certified by the American Nurses Association as a clinical nurse specialist. She received her nursing degree from Misericordia Hospital School of Nursing, Bronx, NY; a bachelor of science in community mental health from State University of New York, Empire State College; masters of science in nursing, psychiatric mental health from Adelphi University, Garden City, NY; and wholistic nursing certification from New York College for Health, Education and Research, Syosset, NY. She has extensive experience in mental health nursing and working with children and adolescents. She has also served on the faculty of schools of nursing at University of Southern Alabama, Boston College and Adelphi University.

Dr. TerkelsenKenneth Terkelsen, MD
Dr. Terkelsen is a psychiatrist. He is a graduate of Georgetown University and received his medical degree from Jefferson Medical College of Thomas Jefferson University in Philadelphia. He completed a residency in psychiatry at Albert Einstein College of Medicine at Lincoln Hospital and a fellowship in social and community psychology at Albert Einstein College of Medicine at Bronx Psychiatric Center. Board certified in psychiatry, Dr. Terkelsen has many years of experience in private practice and in community settings on Cape Cod including Gosnold-Thorne Clinics, Cape Cod Human Services/Cape Cod Behavioral Healthcare and the Cape Cod and Islands Community Mental Health Center.

 

Katherine WalshKatherine Walsh, LMHC
Ms. Walsh is a counselor. She has a Masters degree in Expressive Therapies from Lesley University. She previously worked as a clinician at Gosnold Treatment Center and as a school based counselor. Ms. Walsh has also worked in day treatment and partial hospitalization settings facilitating various therapy groups.

 

Glenn WilliamsGlenn Williams, RN
Mr. Williams has an Associate of Science in Nursing degree from Mt Wachusett Comm College in Gardner, MA. He has 33 years experience in various psychiatric nursing settings including the Visiting Nurse Association, Massachusetts Department of Mental Health acute and chronic care settings, private psychiatric hospital inpatient units, hospital emergency rooms, and crisis intervention sites. He has expertise with dual diagnosis and detox inpatient care.

 

Heidi WrightHeidi Wright, LMHC
Ms. Wright obtained her Masters degree in counseling psychology from Lesley University. She has worked for Massachusetts Society for the Prevention of Cruelty to Children doing outreach with clients ages 4 thru adult and has extensive experience working with those affected by trauma. She has also worked with inmates at the Barnstable County House of Correction.

Dental Providers:

Dr. ManciniJohn Mancini, DDS , Director of Dental Services
Dr. Mancini obtained his dental degree from Georgetown University School of Dentistry. Prior to joining the Health Center, Dr. Mancini was the dental director at Mid-Upper Cape Community Health Center and the Ellen Jones Community Dental Center in Harwich. He helped to create the Cape Cod Dentists Care program, a voluntary group that provides low-cost dental services to the uninsured. Before moving to the Cape, he created a children’s sealant program and established a dental clinic in Fitchburg, MA. He was also in private practice for 27 years.
