HOW TO OBTAIN SERVICES

If you do not have health insurance, we will help you apply for MassHealth, Commonwealth Care or other health benefits. Health insurance enrollment and application assistance takes place on Wednesday evenings from 5:00 to 7:00 PM at its Mashpee location at 107 Commercial Street. This assistance is also available Monday through Friday from 9 AM to 5 PM. An appointment is not necessary. It is not necessary to be a patient of the Health Center to take advantage of this service.

The Health Center’s enrollment specialist will review eligibility guidelines and help you complete the online applications for MassHealth or Commonwealth Care. It takes about 30 minutes and you will receive a response in the mail within 10 to 14 days.

To complete the application, you should bring your last two pay stubs or most recent tax return if you are self-employed, as well as photo identification and proof of citizenship for U.S. citizens. The same assistance is available by appointment during the day at the Health Center’s Falmouth location, at 210 Jones Road, Suite 22. Call 508-477-7090 for an appointment in Falmouth.

MAKING AN APPOINTMENT

You must be a patient of the Health Center to use our health services.

Because of the many people who need our services, we must schedule all patient appointments. We do not offer walk-in service at any of our locations.

Please call 508-477-7090 to make an appointment or for information about becoming a patient. We make every effort to schedule new or routine appointments within an acceptable time frame, but please be aware that we are experiencing a backlog for new patients. Please be patient.

You can download (from the Home page of this website) and complete the Patient Data, Authorization for Treatment, Release of Information, and Health Questionnaire, and bring them with you when you come for your first appointment.
