If you do not have health insurance, we will help you apply for MassHealth, Commonwealth Care or other health benefits. We can help you determine if you’re eligible and complete the application process. Stop in or call the Health Center in Mashpee at 508-477-7090 for an appointment Monday–Friday, 8:30 AM to 4:30 PM. In Falmouth, stop in on Mondays, from 9 AM to 2 PM (Portuguese) and Thursdays, from 1 to 4 PM. An appointment is not necessary. It is not necessary to be a patient of the Health Center to take advantage of this service.

The Health Center’s enrollment specialist will review eligibility guidelines and help you complete the online applications for MassHealth or Commonwealth Care. It takes about 30 minutes and you will receive a response in the mail within 10 to 14 days.

To complete the application, you should bring your last two pay stubs or most recent tax return if you are self-employed, as well as photo identification and proof of citizenship for U.S. citizens.

Because of the many people who need our services, we must schedule all patient appointments. We do not offer walk-in service at any of our locations.

Please call 508-477-7090 to make an appointment or for information about becoming a patient. We make every effort to schedule new or routine appointments within an acceptable time frame, but please be aware that we are experiencing a backlog for new patients. Please be patient.

From the Home page of this website, you can download the New Patient Registration Forms and complete the Patient Registration, Health History Questionnaire, Authorization for Treatment and Health Center Services, and Authorization for Release of Information, and bring them with you when you come for your first appointment.
