Making an appointment
Because of the many people who need our services, we must schedule all (routine and sick) patient appointments. We do not offer any other walk-in service at any of our locations at this time.
We make every effort to schedule routine appointments within an acceptable time frame. Please call (508) 477-7090, ext. 104 to schedule or cancel an appointment. There are a number of same-day appointments available based on medical urgency. Please call (508) 477-7090, ext. 125 if you are sick and need to speak with our triage nurse. For individuals who speak Portuguese please call (508) 477-7090, ext. 151.
As always, in case of medical emergency, call 911 or go directly to the nearest hospital emergency room.
Health insurance
If you change or lose your health insurance, please contact our office and we will update your new insurance information or arrange for assistance to help you re-apply. Please do not miss an appointment because of an insurance issue – we are here to help! Contact our enrollment specialist at (508) 477-7090, ext. 155 with any questions regarding health insurance.

We are welcoming new patients at our offices in Mashpee, Falmouth and Bourne. You must be a registered new patient of the Health Center before scheduling an appointment with a medical provider.

Please follow these easy steps to become a new patient:
1. Apply for insurance if needed
We have staff at all offices to help you apply for insurance.
Health Insurance Application Schedule
It is best to call ahead to be sure someone is available. If you cannot make one of the scheduled times or have any questions about health insurance applications, please contact Joan Dowd, Outreach Coordinator, at (508) 477-7090, ext. 155.
If you are not a patient of Community Health Center of Cape Cod and do not plan on coming to our Health Center for your medical care, you can still receive health insurance application assistance on Thursdays, from 10:00 am to 12:00 pm at the Falmouth Service Center located at 611 Gifford Street in Falmouth (http://www.falmouthservicecenter.org). Other organizations offering health insurance application assistance to individuals are: Duffy Health Center, Mid-Upper Cape Health Center, Outer Cape Health Center, the Community Action Committee of Cape Cod and the Islands (http://www.cacci.cc), and the Wampanoag Health Center. All Senior Centers will help individuals over the age of 65 years old and anyone who is on disability. Falmouth and Cape Cod Hospitals have financial counselors who can do insurance applications as well.

2. Complete New Patient Admissions Packet
Pick up a New Patient admissions packet (available on our website or any of our offices), read and complete all information.
Return your competed registration forms to any office, by mail, or by faxing them to our main fax number (508) 477-7028.
It takes us 2 to 4 business days to enter your information and then someone will contact you, usually within 2 weeks, to help you choose a medical provider and schedule a new patient appointment.
