So that patients from across the Upper Cape have convenient access to our services, the Health Center has offices in Mashpee, Falmouth and Bourne. Hours of service differ for each location. Appointments can be made by calling a centralized number:

Night & Weekend Coverage
If you need to reach the Health Center when we are not open, please call the main number – 508-477-7090 – and you will be connected with our after-hours coverage. Remember, in cases of extreme emergency, call 911 or go directly to the nearest hospital emergency room.

Interpreters
We are able to arrange for foreign language or sign language interpreters. Please let us know in advance if you need an interpreter for your appointment.

Mashpee:
107 Commercial Street

Tuesday, Wednesday & Thursday, 9 AM to 7 PM
Walk-in Care (for enrolled CHC patients only) is available Monday – Friday from 9 AM to 5 PM. No appointment necessary.

Directions: From Route 28, turn on Charles Street at entrance to Marshall’s. At Stop sign, turn right on Commercial Street. The Health Center is 1/3 mile on the right.

Please note that Commercial Street is a new road and there are not many buildings on it. Keep going. YOU’RE NOT LOST!

The Breeze/Sealine bus, which runs between Woods Hole and Hyannis on a schedule, will stop at the Health Center, if you ask the driver. If you’re at the Health Center, call 800 352-7155 for pick-up. Falmouth:
210 Jones Road, Suite 22, Homeport (intersection of Jones Road and Gifford Street)
Monday – Friday, 9 AM to 5 PM

Directions: From Mashpee: Follow Route 28 into Falmouth. At traffic light at Stop & Shop, turn right on Jones Road. Proceed 6/10 mile to traffic light at Gifford Street. Continue through intersection and turn right into the 3rd driveway after the light. This is Homeport and the building is on the left.

From Bourne Bridge: Follow Route 28 to Falmouth. At first traffic light, turn left on Jones Road. After passing the courthouse on the right, look for a grey building on the left with a large “210 Jones Road” sign. Turn left after this building into Homeport. The building is on the left.

Bourne:
123 Waterhouse Road
Monday - Friday, 9 AM to 4 PM

Directions: From Bourne Bridge: Bridge: Take Route 28/MacArthur Blvd south for approximately 1.3 miles. Take right on Paesano Place and an immediate left into #123.

From Falmouth & Mashpee: From Route 28 at the Otis Rotary, take Route 28/MacArthur Blvd. north for 2.75 miles. Cross over Route 28 south at Trading Post Corner (Atlantic Subaru) to Waterhouse Road. #123 is Paesano Place, on the right.
