Welcome to the website of Community Health Center of Cape Cod, formerly Cape Cod Free Clinic & Community Health Center. As a patient-centered medical home, we are a resource for prevention, wellness and quality care, not just a place to go when you are sick. We provide primary care, dental care, women’s health, and behavioral health services at our offices in Falmouth, Mashpee and Bourne, as well as at Barnstable High School in Hyannis.
The Health Center, first known as the Falmouth Free Clinic, was started in 1998 by a group of dedicated volunteer physicians, nurses, counselors, and lay people who came together to provide free health services to uninsured residents of Cape Cod and the Islands. Today, we are a non-profit Federally Qualified Health Center, providing access to health care for children, adults and families. We welcome patients with MassHealth, Medicare, Commonwealth Care, private insurance, or no insurance at all.
Our programs and services are provided by a caring team of physicians, nurse practitioners, physician assistants, dentists, nursing and dental staff, mental health counselors, social workers, and administrative staff. In addition to our core services, we have developed special programs to meet unique needs in the community, such as insurance enrollment assistance, primary care for veterans, teen and adolescent services, youth suicide prevention services, and referrals for specialty care.
Through all our growth and change, our mission remains constant: to improve the health of the patients and the community we serve through the provision of comprehensive, high quality, compassionate health care to all, regardless of ability to pay.
