Services, including primary and preventive care, dental care, mental health services, coordinated case management, and prescription assistance, are provided at locations in Falmouth, Mashpee and Bourne.

Family Practice and Internal Medicine Physicians
CCFC&CHC is seeking a full-time, board certified/eligible family practitioner and internal medicine physician for our Mashpee and Falmouth locations. We offer a flexible schedule, no hospital and full compensation packages.
Nurse Practitioner/Physician Assistant
The Health Center seeks a nurse practitioner and physician assistant to provide comprehensive primary care. Current MA licensure and strong clinical, communication and computer skills required. Flexible schedule available.
RN/LPN/Medical Assistant
Experienced nursing professionals needed. Strong clinical, computer and communication skills required. Bi-lingual and multi-tasking a plus! F/T and P/T positions available.
Dental Receptionist
Full-time, experienced dental receptionist, 40 hours per week to provide assistance to the Dental Director and Dental Manager. Prior experience with Dentrix software a plus. Bi-lingual and strong clinical, communication and team skills desired.
Counselor/Social Worker
LICSW or LMHC - part-time to work with families and children in new Family Resource Center. Strong clinical, communication, computer and team skills desired.
