Community Health Center is a proud member of the C3 Accountable Care Organization

An ACO is a group of health care providers that work together and are accountable for both the total cost of care provided to their patients and for the quality of the care provided.

CHC joined with 24 other community health centers to form a new ACO called Community Care Cooperative (or C3).

What Makes C3 Unique

CHC and the other health centers of C3 share the same mission and values to support low-income population.

Most Massachusetts ACOs are hospitals or health systems. C3 is the only health center-based ACO and one of only three non-system based ACOs in the state.

As an ACO, we are not solely focused on the volume of care we provide. We are concerned with ensuring everyone gets what they need throughout the entire health care system.
