Community Health Center is a one-stop destination for almost all of your healthcare needs.  We are a complete medical home that offers primary care, behavioral health care, dental care, gynecology, optometry, substance use disorder services, on-site affordable prescriptions, x-ray and ultrasound services, barrier assessments and assistance, Veterans care, and many other specialized programs.

Our focus is on providing the very best health care, as well as providing programs and education that help improve the overall health status of everyone living in our region. CHC is a non-profit organization that cares for all patients, regardless of ability to pay or insurance status. If you don’t have insurance, our experts will help you enroll so you can have access to most convenient and affordable way to stay healthy.

No one will be denied access to services at Community Health Center due to inability to pay, and there is a discounted/sliding fee schedule available based on family size and income. Community Health Center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals. This Health Center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).

Community Health Center provides an on-demand, mobile video connection to medically trained language interpreters, including American Sign Language. We use iPads on stands with wheels which allow us to bring the interpreter to non-English-speaking patients wherever they are within CHC.

The medical providers at the CHC are physicians, nurse practitioners and physician assistants. Nurse practitioners and physician assistants are healthcare clinicians who work under the direction of a physician. They perform exams, diagnose and treat acute problems, order diagnostic tests, and prescribe medications.

Our providers are all part of the care teams, which also include nurses, medical assistants, counselors, and registration and referral specialists. They work closely with pharmacy and dental staff to fully coordinate patient care.
